Hyperarousal and Hyperviligance

I have lived with insomnia for many years. I remember as a young teen sitting in my parent’s living room watching the slow dying glow of a log burning in the fire place. It was the middle of the night. I was told as a very young child that I used to sleep walk and I actually fell down the stairs. I can remember when I was 30 the sleep walking started up again and I also had other sleep issues. The biggest issue I have now is related to PTSD. This is when you wake up in the middle of the night and cannot go back to sleep for hours. But there is more to this than a bit of anxiety. It is hyperarousal.

Hyperarousal can be defined as the state of being aroused, on guard, being overly alert and perceptive. The person who is suffering from hyperarousal is actually in the middle of the ‘fight-or-flight’ reflex that occurs when the brain sends out signals of impending danger or feels threatened. It puts the body on guard – ready to run away or stand and fight. I suffer from hyperarousal during the day as well. But in the middle of the night, this beast rears its ugly head and I am struggling for hours. If I do fall back asleep, I usually end up with a nightmare of some sort. All this is chemically regulated in the body. I do not know what exactly the trigger is all the time for me, but it happens when I am in a state of stress. I can perseverate on the smallest detail and by the end of an episode; I will have worried myself into a real frenzy.

Two weeks ago I actually got up and took my blood pressure and the reading was extremely high. But it was high all week from the stress of leaving my job. The physical condition I was in from the pressure of work and the all BS going on played havoc with me. But even with being away from the situation, I am able to find other things to fret about.

Hyperarousal also causes me to erupt. I can over react to the smallest things such as simply dropping something. I get frustrated very easily and have an over the top reaction. For me, it is compounded with the high level of pain I am in from my psoriatic arthritis. Unfortunately, it is a nasty loop. I get mad from being in pain and limited in my mobility and then I get a flare from being upset. They feed each other and around I go.

Compounding the situation is the sister reaction to hyperarousal which is hypervigilance. That is when you are always on alert. This is when you constantly are scanning for threats. When I was first diagnosed with PTSD, the therapist said I was like a trapped animal. I never turned my back to him. I constantly watched every move he made, especially with his hands. I think I am better but when I get threatened or perceive I am being threatened, I go back to being hypervigilant. I realize now that the past couple of years being in the toxic work environment I was in really set me back. A result of being hypervigilant is being exhausted. So I fall asleep very quickly but wake up in the middle of the night. On average, I sleep in two to three hour blocks. A good night is when I can sleep with a four hour block.

I write this not as a venue to complain but to share for someone who is going through similar things. There is comfort in knowing that you are not alone. I am planning on talking with my doctor about this if my sleep does not get any better. They recommended last year I seek retirement. It has been a long haul getting away from work and the nasty people I worked with. I have had some things going on in my home which has been concerning so I have not been able to totally calm down. But I am physically more active at home and that helps. I also need to get back to a strong practice of meditation again. That really helps. I already feel better and I know I am healing. But it is a slow path.

Trauma informed

I have spent a great deal in the past months seeing a variety of doctors for various reasons. At every visit, every time, the patient care-tech brings me back to the visiting room to wait for the doctor. It is their responsibility to weigh, check blood pressure and ask a few questions. One of those questions is the medical community’s answer to being trauma informed. It misses by a huge mark.

Being trauma informed simply means being sensitive to a patient’s history or the possibility of the patient being in a dangerous living situation. We are talking domestic violence, and all forms of abuse: mental, physical and sexual at any age. Being a trauma informed  means everyone on your team from the receptionist to the doctor is sensitive to how they treat a patient. Some may say this is over the top but the concept is very important to the true wellness of all patients.

For example, the patient who keeps coming back with bruises or injuries from falls. Very suspicious if the patient is only forty. Maybe not so for someone who is eighty. But in the case of the eighty year old, she or he may have a care giver who has been frustrated in having to provide care and pushes them to make them move faster, or hits them when they spill things. And the patient is not going to do anything about it because the next step is being forced into a nursing home. They are living in an abusive home and are trapped. They certainly are not going to be forth coming about it.

Patients who have experienced domestic violence of any form will react differently to the way someone approaches their safety bubble. What I mean by this is that everyone has a comfort zone for how close people can get and how comfortable they are being touched. Some people are huggers and touchers and love to get close to anyone, including a stranger. People who have been abused have much larger safety zones and very often do not want to be touched. This not wanting to be touched can range from the patient being able to tolerate it by disassociation or the patient who has a panic attack at the mere thought of having to be touched, which often leads them to not go to the doctor when they really need to.

It an attempt to becoming trauma informed, our local medical center has all providers asking about the safety of their patients? It is a useless attempt to meet a standard. First of all, it is not the doctor who asks this question. It is the care-tech, the gum chewing little twenty-something who is trying to beat a record of some kind by seeing how fast she can get patients in the rooms ready for the doctor. They change regularly and even so, I doubt highly any patient who is in a domestic violence situation is going to open up to that individual as if they are going to be able to do anything. AND… it is none of their business. The lack of sensitivity to the situation of an actual abuse situation is very typical. Unless there has been some form of specific training done with people who are asking that question, simply asking the question almost makes it worse. What are they going to do if a patient says, “no I am not safe. My husband routinely comes home drunk and beats the shit out of me and then rapes me.” The response to that can make or break the patient. The care-tech’s only course of action would be to type ABUSED into the chart. That works!

I would love to know how asking this question, “do you feel safe in your living environment” is the proper opening for a patient to disclose something so humiliating and degrading as being abused. And how many providers are even trained to handle the situation past the physical? Do they know who in the community is providing help for domestic violence? But the bottom line, most patients will not jeopardize themselves by spilling the beans about their crappy home life in a brief visit to a doctor, especially if there is not a relationship built over time with that doctor.

But being trauma informed goes beyond asking that inane question. It is things like how the patient is treated by everyone; from the rushed handling of the care-tech to the actual doctor. Patients who are victims of abuse will react from the way they are placed in a room and then left abruptly to wait for an extended period for the doctor. Letting the patients know if the doctor is running late, or checking in on the patient to see if they are ok would go much further to calm a nervous person. Having the doctor explain what and why they are going to do something BEFORE doing it will help also. Even the simple act of having them listen to the heart can be traumatic. Some doctors will do it over clothes, some doctors reach right in without warning for skin contact. Sometimes there may be a need to hold the patients head while examining their throat for example. This simple act can terrify someone who has had their head restrained in an abusive act such as forced oral sex. Lying down on an examining table makes anyone vulnerable, but for an abused patient, it is excruciating.

For some patients, the act of disrobing will send them back to a place of past abuse. These patients need a sense of safety which throwing them into a sterile, brightly lit room and demanding they disrobe behind a flimsy curtain does not provide. Then they have to sit in anticipation of being probed on a ridiculous scary examining table sitting in the middle of the room as if they were a piece of meat shivering in a paper gown. Would it be so absolutely terrible to provide a soft blanket to comfort and to also help with limiting the exposure while being examined? Something that simple is being trauma informed.

The patient is brought back to the exam room and sat down after being weighed. For some, being weighed is very traumatic. I know that doctors need the vitals but unless you are suspicious of your patients and do not trust them, can you not simply ask them what they weigh? Or make it part of the exam in private and not in a hallway where everyone is walking by. The scale calculates and the care-tech yells out the reading like everyone needs to know….and you still have your boots on. I skip it and refuse. Unless I am there for weight related issues, I know what I weigh and I will tell the doctor if they ask.

Health care workers are highly trained. But in the area of trauma informed, there is much to learn. This study goes hand in hand with a comprehension of the ACE study, which identified the link of abuse to chronic illnesses. I have asked all the doctors who I see if they are aware of the ACE study, and none of them are. I am going to leave a couple of articles for them to read. (see below) We need to work on this information being disseminated and so if you found this helpful, read and print these out and give them to your doctors.

In hot water

fogfalls   This sound really egotistical and it is not what my intent is. I am sincerely amazed how intuitive I can be. It is not always a great thing but it has kept me alive. This may seem like a small issue to some, but what happened this last week tells me it is important enough to share. Maybe someone else is like this and does not realize it. I am talking about stress and what it does to you.

About a month ago, I had my heating system annual tune up and cleaning. The guy who came to the house was very nice. The same week, my mother-in-law was in the hospital dying. The day he came, we were all supposed to meet at the hospital to talk about a care plan. I wanted the guy to hurry up and finish. I went to check on him and he starts telling me my system needs to be drained down and a new valve needed to be installed; the same valve that was replaced two years ago. Every time they come out, they find something wrong with the system. When we turned the heat on, it dripped two drops. He said he was going to adjust the pressure and some other things and then forty minutes later he left. They would call me to set up an appointment for the next work.

As the weeks went past, there was nary a drip. The system is a gas fired boiler that circulates hot water throughout the house. The heat is not drying as force air and is very cost efficient. And boilers last for decades. The parts that support the system do not. Since I have been here fifteen years, I understand that things need to be updated. The system is very quiet except for an occasional tick of the baseboards.

What was happening when the system kicked on was this rush of water coming into the system that sounded awful. It was very loud in the kitchen and the first time it happened it scared the crap out of me. I thought we sprung a huge leak. I went downstairs and all was well. We were not home a lot the next weeks and the weather was warm so it did not happen too often.

But every time it did, I would tense up so bad. Stories ran through my head of imminent disaster. It was amazing how loud the rushing water sounded. I thought that maybe he put the pressure down and that eventually the water would stop doing it. But it didn’t.

But my body flared. The intense reaction was so visceral. My home was in danger. I love my house. It is my safe place. I was not sleeping and a lot of other things were happening in my body. My morning glucose readings were very high. On top of the boiler, the garage door opening system broke and needed repair. I discovered that on Tuesday of last week when I came home and the safety eye was laying on the floor. The door does not close without it facing the other eye.  Then on Wednesday, the ice maker crapped out.

Luck was totally in my corner. The door people had an opening on Friday and could come out to fix it and tune up the opener. My husband was off and home. Perfect. He knew I was upset about the ice maker so he called someone and they could come on Friday too. We also had scheduled a tune up for my car on Friday, which was supposed to be no big deal. But you know how that goes.

I was so freaking frazzled that by the end of the week, my body literally was on fire and I had a diverticulitis attack. I was not sleeping and my hands and hips and back were flaring so much I could hardly walk. I went to work in the morning, after dropping my car off with the intent of picking it up and being home when the icemaker dude was coming. Since I was going to be home, I called the heating people and after a run around, they were going to send someone out as well in the afternoon.

The heating chap was a rather nice young man and he listened to what was going on. I told him the story of the valve and he of course was thinking his fellow worker was correct. But he heard the rushing of the water and looked at the gauges and said there was not enough water in the system and that was a big issue. He wanted to at first drain the system by going all over the house and opening up things… and I thought I was going to be sick. Then he put some water into the system and he thought the gauge was broken. He drained some water with a hose he had and a bunch of other things. He had so many scenarios of what was wrong and he tried many things. Nothing was wrong with the gauges. There just was not enough water in the system. He filled it back up and then went out to his truck and called the guy or the office. I went into the other room. He came back in still on the phone for a while. Finally, he called for me. He looked funny.

Seems I was absolutely right. The guy before drained water out of the system and reset the gauges improperly. The poor boiler was trying to operate without the right pressure or water. He could have ruined the system. There was nothing wrong once he filled it up correctly and reset the gauge. It has been quiet, no drip and is keeping us toasty. They offered me a maintenance agreement and gave me such a deal on it as an apology that it paid for itself. I get two years of free tune ups. Good customer service.

But what amazed me is how much my body calmed down. I spent the rest of the weekend in a much better place. I slept better than I have in a month. My glucose readings dropped fifty points and it is not from eating better, that’s for sure. My physical reaction to the loss of safety in my home was not controllable because I did not even know how bad I was until the things were fixed. I feel so vulnerable because I do not know how to fix the mechanics of my house. I hate being fleeced.

The car was fixed and only what I needed. The garage door works terrific and he fixed the issue of why the eye fell off so it won’t happen again. It did once before and I got it back on. The icemaker had to be replaced but I love having ice so it was worth it. The guy who came out was very nice and lives nearby and fixes all kinds of appliances. It is a good feeling of having someone like him available. He was honest and fair. All is well and there is a safe feeling again in my house.

Now, if there was only some way I could fix what happened on Tuesday with the elections……..

 

 

 

 

Midsummer Dreams

The light has shifted ever so slightly in the garden at night. I have been doing an epic battle to keep everything hydrated. But even so, things are crisp or wilted. My glorious ferns are gone and the grass is brown and crunchy. The trees in their deprived state are dropping leaves early. I fear for how brown our fall will be this year.

I am not good with the heat. The other night, the little air conditioner in our bedroom could not overcome the heat and it was still 80 in the room. I find myself longing for a storm or two. I realize I could never live somewhere that was sunny perpetually.

I spend my last hours of the day in the garden readying from my Kindle. It’s so dry that the bugs and mosquitos are less and so the light does not attract them. I finished a non-fiction book intended to reflect on what it is like to live with PTSD. It was a story of woman who was raped by her boyfriend over a period of times when she was 16. She lived with his threats and never told anyone. The book revolves around her growing up and dealing with the ramifications. She is splinter from her family, her own choice, and ends up as a disenfranchised woman living in a hovel.

Although the book explained about triggers it was not a good representation of what it is like to live PTSD. The woman in the story goes to a psychiatrist and is “healed” by simple breathing methods and other mindful exercises. She meets a man and life goes on off into the sunset. Although the authors attempt to help bring awareness, I think she misrepresents the truth.

People do not ever heal from PTDS. They learn to cope. And while some are better than others, it still can rear its ugly head at any moment. Triggers come from everywhere. I was reading where a female soldier (nurse) who had PTSD after being in Nam was fine for years. Then she moved to a new area to work at a hospital. She started having horrible flashbacks and attacks. She could not figure it out when on a very still night she heard the sound of a chopper flying to the landing pad at the hospital. It was not the first flight since she moved there. Normally it was noisy with other ambient sounds and this was so subtle she never paid attention. But her ears did. They heard and she would start to have panic attacks.

The shift in light for me is a trigger. I am so sensitive to it that I am aware before it really gets to the point where it bothers me. Something about the afternoon light before sunset in the fall and winter makes my chest tighten and other sensations. It has to do with sunset around 5 pm. This was the cocktail hour when my parents would barricade themselves away from us and start drinking. Yes, this still affects me forty years later. I am aware of it and can normally deal with it. But I still get a stomach ache and my mood shifts.

This morning, as many Sunday mornings, I sleep an hour or two longer than normal. It is very common for me to have nasty dreams but there is a prevalence of one reoccurring situation and it often plays out in these stolen moments of extra sleep. I have no idea why and I cannot control my dreams. Even after being divorced for over 13 years, I still have horrible dreams about the way I was treated. I am not going to dwell by explaining this as I need to let the dream I had this morning go. But my point is PTSD does not just end.

I know what I have to do and will tend to it. The summer ending is always hard for me. We are off to the River for our long extended stay in a couple of weeks. That will help me to focus and ground. There needs to be more awareness of PTSD, and not just for Vets. It affects many people in many ways.

There are times in my life when I think why? Why do we as humans struggle so much just living. Was it like this since the beginning of time? Is there anyone out there who truly has no stress, no difficulties and breezes through live without conflict? Or is this what human nature means?

Being diagnosed with PTSD was the biggest ah-ha moment of my life. So much finally made sense in my world. Diagnosis means being aware and of all the medications and treatments, being aware the one thing that matters. Because I believe there is no cure. There is only living with it.

I have done as much mindfulness and meditation as humanely possible. I have sought treatments from cognitive behavior therapy to AAT and other forms of energy work. I am not saying they do not “work” but they do not cure. There is no cure. There is acceptance and awareness.

Last week, something tripped me over the weekend. I have no clue what. That is one of the tricky parts of having PTSD. You do not need to know what trips you off, it just happens. By Tuesday, I was in a full blown attack. During the day, I am anxious about stupid things like driving and loud noises. I jump at the slightest provocation. I am bit short in patience. But the telling sign for me is I gain a few pounds that won’t come off. And I have not changed what I am eating. My body pain level is extremely high and I have sharp pain in certain spots like my neck and shoulders which were tight as a trampoline. I cannot take a deep breath even when I doing breathing exercise. But the most telling is I do not sleep. Tuesday night I had about two hours of sleep. The rest of the week my sleep was fitful and full of nightmares that stay with me during the day.

On Wednesday, I was asked to do a presentation for the whole management group on a project I am leading. I then have to drive 40 miles one way to deliver the same presentation to another group. Just as I seemed to becoming down a bit, or maybe just calmer because of lack of sleep, this set me off to an even higher level of anxiety. Just writing this made me catch my breath. It is not that I am nervous about presenting. I just hate presenting to this group. Hate it! It is like they are sitting there with bared teeth waiting for you to make a mistake or in total judgment (which they are). Driving is another big time stressor. By the end of the day on Wednesday, I was a mess. The rest of the week I was a walking nerve fretting about it.

On Friday, I sat down to finish the PowerPoint and having accomplished that helped to calm me. Later at home that night when I was somewhat at peace, I went into my work email. Big mistake. We have a Coach from the med center who is overseeing the program I am presenting on. I am supposed to be learning from her because when she leaves in a couple of months, I am supposed to fill in. She never likes anything anyone is doing. We have had people actually go to our CEO and complain about her. She always has something to correct me on. I think she is a terrible coach. Sure enough, she hated the PowerPoint and said that she thought that only three of the fourteen slides had value. I just burst into tears and sobbed for a while. I was exhausted from the week and then this.

I did something I usually do not do. I fought back. I forwarded the email to my boss who is aware of the situation with the coach. I also sent an email back to the coach and said that it seems I never seem to get things right with her and this was making me very anxious. My boss wrote me back and said the PowerPoint was exactly what the administrators wanted and she liked it. I have not heard anything from the coach. But I felt I stood up for myself and that in itself is powerful. Shame is often a big part of having PTSD and so to stand up for myself was a big deal.

My thoughts for this week included feeling trapped. That is a sensation I have lived with forever and it is not because of any one situation. It is from years of emotional and mental abuse. I felt feel trapped in a job that is frustrating and limiting and often does not bring much satisfaction to me. It does not make me feel like I am contributing to the world. I am trapped as I need to work and the thought of switching jobs is worse than living with it….which is why I stayed in a shitty marriage for 27 years. Fear.

I hate the thought of presenting to this group because of their critical judgment. This is all management including the ones who are clawing their way up the chain and would do whatever they could to push someone out of their way.  This group also has staff that have been there a while and like the way it was always done…. Change agents are not welcome.  Fortunately, there are less of them now. There is a woman who will be in the audience who looks and act so much like my mother and I have had a terrible time with her. Her reputation is that she looks for the holes or mistakes in presentations and will pounce on any weakness. She constantly interrupts to ask for “clarification” and asks a million questions. Everyone knows this about her and it is a big joke, but it is also terrifying when she is grilling you. It does not help me that I have her position which she held for 12 years and she feels I am inadequate because I am not a nurse. When I first started, she had a posy of peers who felt the same way and made my work life difficult. A few of them have left or retired but she perseveres on.

The thing is… I know this is only fifteen minutes max of my life. I know this will pass. I know I will be fine driving 80 miles as it is not snowing and I take back roads.  I know that by Wednesday night, all this will be over. I know all this. I fight to be in the present moment and not worry about the future. I know perseverating on this only adds and does not help. I am writing as another act of trying to cope. I am employing techniques that have I learned to calm. I am trying to think before I react, which is another sign of being in the throes of PTSD. My reactions are over the top and visceral. I am using all the techniques and skills I have been taught to cope. They are helping some.

But sadly the truth is I just have to wait for it to pass. I can only hope that nothing else triggers me and I can deregulate down to a more homeostatic level. I know I will emerge once again out of the rabbit hole.

Resolutions

dog1  It has been almost two months that I have been going through a “thing.” I am on the other side of it now and thought I would share. To help illustrate my thoughts, this young golden was photographed up at the docks in the TI Park, 1000 Islands. It is one of my favorite places.

Just before vacation, I took my shot for my Psoriatic Arthritis. I have been on Simponi for about eight months with some relief, but not great. This time, I had a reaction at the injection site which is something I have never experienced before.

At work, I was having issues with one of my employees who is enthusiastic, but a royal pain in the butt. She was working on a project and had managed to take a small pilot and manifest it into a major fiasco that was causing issues everywhere. She also was racking up hours that I did not have budgeted as she is per diem. I decided to have a meeting with her and my boss to get some clarity on her role. She went absolutely bat shit in the meeting and accused me of all sorts of mismanagement. She was almost screaming and my boss said nothing. And her main point was that I did not understand because “I am not a clinician”.  Anyone who knows me knows this is a hot button for me. I sat in the meeting listening to her accused me of cancelling meetings with her and not responding to emails and more lies. I felt my chest tighten and thought, “holy shit, I am having a heart attack.” The pain was sharp and frightening. I disengaged from the meeting and let her rant. I figured my boss would see through her lines of bull. But I was extremely upset.

The next day, and the last day before my vacation, I went to see my boss and expected she would say that this girl was nuts. Instead, she lambasted me with “I need to communicate with her and manage her better.” I was stunned.

By then, I was feeling really crappy. I was going to my favorite place in the world and I was anxious and upset. My gut was killing me. I never truly let go of the meeting and situation at work and at night, I was having heart flutters and anxiety attacks. During the day, I felt bloated. I knew my blood pressure was up. My stomach and gut was flaring with pain.

When I came back, I emailed the employee and asked that she validate everything she accused me of. She said in the meeting she had emails of me cancelling her and using a “tone” with her. I went through my stuff and found nothing; I have been keeping every email she ever wrote. I spoke with my boss who apologized when she came back from her wedding and vacation because she realized she had inadvertently validated this person. With both knew that was disastrous.

Three weeks from taking the shot, my blood pressure was still extremely high for me and I was dizzy and felt out of it. I was still having some flutters and tightness, so I called the doctor. He ran blood tests on everything and many of my levels were extremely high, including my sugar. (HA1C) By the time I saw him, it was a month from the shot. He decided to put me on a beta blocker to help lower my still high blood pressure. It was a tiny dose of metoprolol.

But OMG, did I have a reaction from it. First I had insomnia, then that went away. Then I was nauseous every morning and the short 20 minute ride to work was a nightmare as there are no bathrooms. I had several close calls. Then the diarrhea was all the time. I was gaining weight every morning which was counter intuitive to the output I was having. I felt like…pardon the pun, shit. I was going to see the doctor again soon. I had all the tests redone and then decided enough was enough and weaned myself off the drug. The results from the lab indicated everything was fine. After going off this drug that was supposed to help my cardiac issues, they all stopped. MY BP at the doc’s visit was 124/80. MY HA1C was back below my prior visits and so were most of my levels. Problems solved.

And this issue with the employee? She went off on a tangent and was spreading lies about me to people. She never validated any of her claims and when I met with her and again called her on it, she said, “oh, let’s not open that all up again.” She was racking up staggering hours and when I called her on them, she would push back with more bullshit. The results of her craziness combined with putting her nose in places it did not belong ended up with another meeting with three directors and myself and my boss, the VP of HR.

Finally, after almost two years of her torment, she will be put on a plan of improvement that will be so restrictive that she will probably quit. She will be mandated to remain solely in the position she was hired for, which is an instructor and abandon all other pursuits. But if she does not and continues with us, she will surely cross the line in a very short time and then we will severe her from the agency. She will have to eat crow big time because she is facing insubordinate charges but will remain under my supervision. Finally, someone heard me. One of the directors she went to with her garbage suggested she be mandated to go to EAP, for psych support. This employee has burned all her bridges.

And me? I am feeling like my old self for the first time in a long time. Was it just the shot? I do not think so, although a big contributor. I think it was a lot about not being heard and validated. It brought back many feelings from my childhood where I was invisible but took a lot of punishment for the sins of my siblings. I hate when people dismiss me, or say I am over reacting. I hate when I see people getting away with things because they lie or are weasels. I am a good manager and a fair one.  I have never had such a renegade employee in all my thirty plus years of managing people where I had so little control. The stress truly was affecting me. PTSD, the gift that keeps on giving.

 

I am a sinner!!!

“I am a sinner. I have done something so egregious in my life that I deserve completely the issue at hand.”

This is how someone who has PTSD brought on by childhood trauma thinks. This is how a woman copes with the reality of living within an abusive relationship of a spouse or family member. This is what the perpetrator tells them and when it is someone you are supposed to trust, you take it as gospel.

The lesson is learned early when in innocence, the child or woman stands up to the inflictions and is beaten worse or punished even more cruelly. There is guilt placed on them that they are so flawed that their judgement is totally wrong. When you add isolation, which often happens, the person is so humiliated because they are led to believe they have caused their own situation and it is their entire fault. So they submit which is why so many people end up either dead or remain in the relationship.

Children often have no escape. They hear nothing but ridicule and criticism. They are abused mentally, physically and sexually by members of their closet trust unit, their family. This circle of constant wearing down of any self-worth is worn like a cloak and the energy given off to others in the world is a reflection of this lack of confidence. People treat people as the person treats themselves. And this only perpetuates their loss of confidence. When all hopes of a bright future are dashed at the malicious behavior of others, the person mentally gives up.

What happens in the body is another story. The body is designed to protect us. I have written a lot about the reaction of trauma and there is a ton of information on the physical deterioration of the human life form from trauma whether it is a one-time happening or many. Now repeat this treatment for fifty years and then ask, who could possibly have a different outcome of low esteem and self-deprivation? How could the body after all those years of cortisol floods and other hormones and muscles in a permanent clinch all the time not affect the health of someone? This is my story. It is not about sympathy and it is very difficult, more difficult to talk and write about than anything I have ever done because you see….I am a sinner. I still in my subconscious think I deserved everything that ever happened to me. And so the manifestation of chronic illnesses is only another reinforcer.

It is about how obese patients are treated by the medical community. I wrote a long reply to that but it made me come to grips with something currently in my own life.

A week or so ago, I started to feel like absolute crap. We were headed out for vacation; I was able to quell the uneasiness of my ickiness with the anxiety of the trip. I always am anxious to some extreme, but going away is very difficult. I lose what little safety I think I have. Once away, I still was anxious and I thought it was about work. It is what I do. I find a reason for something so I can deal with the guilt of not being perfect. Although we had a splendid time, I knew something was wrong Big Time. We got back and the symptoms did not get better. If anything, I felt worse. I had my nurse educator take my blood pressure and sure enough it was up there, more so than it has been for a long while. I decided to look at my glucose, and yup, it was up. I took readings for four days and it was constantly up there.

I did a very difficult thing. I called my doctor and made an appointment and I asked him to order all sorts of blood work. On Friday, the lab tests came back and there was nothing significant in the findings. I was within a range I had been in before. This summer had been a whirlwind of barbeques and going out every weekend and enjoying ice cream cones while sitting by the canal. And I just came back from vacation where we treated ourselves to what-ever we wanted including shell fish. I had brought all my numbers down when I was on Weight Watchers and because of the change in eating habits, I actually as mindful on the trip and not a crazy fool. I only gained four pounds back from which meant I was still 20 pounds lighter than where I was less than a year ago. Just knowing that made me feel better.

This morning, the last test came back (urine) and it was off the chart. From what I can tell, my kidney crashed again. I had taken a shot (Simponi for my Psoriatic Arthritis) the Wednesday before we left. I had a reaction at the injection site I have never had before (I have had seven shots of this product) I noticed the next day that I felt like my blood pressure was up and I was very stressed. As I said, the anxiety of the trip and so forth. On the trip, I was not drinking water as much as I normally do because of how much I have to pee every five minutes and you can’t do that when you are traveling. I also consumed restaurant food which has a ton more salt than I use. But according to what I have read, I should have edema, and I don’t. There is also the possibility that the test was contaminated and the outrageous reading is inaccurate.

But here is the point of this long epistle. It took me four days to call the doctor. Why? Because in my mind I am totally blaming myself for whatever is happening. I am in a state of being triggered from guilt and the fear of oncoming punishment. For me, having to go to the hospital for more tests makes everything in my body tighten to the point of almost being crippled. I am sure it is adding to the heighten BP. Worse is the treatment I know I will receive. My GP is not like that. But this will surely mean a trip to a nephrologist and/or endocrinologist. My last experience was beyond awful and I wrote about it in Victo’ s post and I have written before about it. Even the nephrologist I saw never talked to me about kidney disease, but only about he bet me I would be so much healthier if I lost 25 pounds. HELLO??? I did and this happened anyways.

I feel in the depth of my soul like I have been captured like a caged animal and I am trapped. Now I will be paraded like a circus animal where people can poke fun and ridicule me. They will tell me that this is all my fault because I am fat and from the front desk to the person who will have to weight me, they will treat me terrible. It is an awful feeling. But I know I have to do something. My fear at one point was so strong that I almost said “screw it. I have to die of something.” Then my new found bravery came into play and I am facing this head on. First asshole who says something that this is my fault because I am fat is going to get an earful from me. I wish I could come up with a card to hand out starting with, “ you are an uniformed asshole! IF you only knew…..” The other thing that this and the conversations on Victo’s blog did was reinforced my desire to continue to work in trauma informed studies. I completed a certification last year in working with organizations to be more trauma informed. There is another longer certification in becoming a trauma informed specialist and I am going to go for that this year.

I know that the fear of seeking medical help is extremely common. The level of anxiety brought on from illness for people with PTSD from any form of trauma is unmeasurable. It is a major cause of suicide. It is only in recent years that PTSD is acceptable for people in the military. It was not uncommon and is still prevalent for soldiers returning to suffer major uncorrectable illnesses because of PTSD. They try to treat the illness and not the whole situation. There is still a stigma of being a weakling. People with childhood trauma and domestic violence victims are very much not understood or even accepted. Countless doctors do not do a trauma screen to see the route of diseases started back when they were physically abused when they were five. And until we stop  degrading and humiliating the victims, they are not going to speak up.

I am going to add to this, although I know this is going to be another post at another time. There is a lot of work out there dealing with people with PTSD and downregulating the body reaction to it. There is ton of stuff on mindfulness and meditation and sensory and energy work and other therapies. I have spent a ton of money and years on trying to find a solution. Although it has helped, some more than others, I am convinced that this is not the cure. I believe there is some so deeply engrained in the body that no one has actually discovered and is able to truly turn off. They can dim it, but not eradicate it. And, like anything, it is hard to say that this too is a failure. But this is all for another time.
